Dr. Joseph Purita serves as Medical Director of PUR‑FORM, where he oversees the clinical protocols that govern every treatment offered through our telehealth platform. With decades of experience in regenerative and longevity medicine, he sets the standard for how care is delivered, reviewed, and supervised across our practice.
In his role, Dr. Purita is responsible for the design and approval of treatment protocols, prescription oversight, and the patient-safety guardrails that every member of our clinical team follows. No medication is dispensed without physician review, and no protocol reaches a patient without his clinical sign-off.
His philosophy is plain-spoken and evidence-based: keep the body functioning at its highest level through care that is measured, monitored, and honest about what works. That standard shapes every consultation conducted through PUR‑FORM.

Shipped Across Florida
Once approved, treatments are dispensed by our partner pharmacy and shipped directly to patients statewide, discreetly and on schedule.

PUR‑FORM provides telehealth services exclusively to residents physically located in the State of Florida. Treatments are prescribed and shipped within Florida only.
