The Process
From intake to your door, in five clear steps.
Every step is supervised by a licensed Florida physician. Here is exactly what happens, in order.
Step One
Complete Your Intake Form
You start online by completing a confidential medical intake form covering your health history, current medications, goals, and the information the physician needs to evaluate you safely.
Step Two
Telehealth Consultation With a Physician
You connect with a licensed Florida physician via telehealth. They review your intake, discuss your goals, and determine whether treatment is medically appropriate for you.
Step Three
Prescription Issued, If Appropriate
If the physician determines treatment is right for you, they issue a prescription. If it isn't clinically appropriate, they'll tell you plainly, because a prescription is never guaranteed.
Step Four
Medication Shipped to Your Door
Your medication is compounded by our licensed pharmacy partner and shipped directly to your Florida address, discreetly packaged. There is nothing to pick up at a pharmacy.
Step Five
Ongoing Provider Support
Your care doesn't end at delivery. You're monitored throughout your program, with provider check-ins and the ability to adjust your protocol as your needs change.

PUR‑FORM is licensed to provide telehealth care in the State of Florida. To become a patient, you must be a Florida resident, physically located in Florida for your consultation, and able to receive shipments at a valid Florida address.
What you'll need to get started

- You are physically located in Florida during your telehealth consultation.
- A valid Florida shipping address is required at checkout.
- You are 18 years of age or older.
We are unable to consult, prescribe, or ship to patients or addresses outside of Florida. Orders with a non-Florida shipping address cannot be fulfilled.

Your consultation happens entirely online. The physician reviews your complete medical picture before making any clinical decision, so there's no rush and nothing is automated.

Your treatment is prepared by our compounding pharmacy partner in accordance with state and federal regulations.
Shipped directly & discreetly
Medication ships in plain, protective packaging to your Florida address, tracked from our pharmacy to your door.
Refills managed by your provider
Ongoing refills are reviewed and authorized by your provider as part of your monitored program.
There is no in-person pharmacy step. Treatments are not picked up at a retail pharmacy; they are shipped to your Florida address only.
